New Client Intake Form

If you are a new client, please complete this form and bring it along to your first session.

General Liability Release Form

This is a standard release form of liability for our massage therapy work together. This form is only required for your first visit if you are a new client.

Intended Health Benefits Form

This form will help you clarify your goals for your overall health and vitality, including our time together.

Health Intake/ Consent Form

You are required to have these two forms for your first session following the Covid 19 outbreak. If you have filled out the Health History form within a year and have not experience any changes in your health, you only need to bring the second page, consent form for your first visit back.

Covid 19 Questionnaire

Due to Covid 19 this form will be required at the time of ever visit until further notice.

Minor Liability Release Form

This form allows a parent or legal guardian to give permission to perform massage on a minor client. This form is only required for your first visit if you are a new client.

Corporate Massage Contract

If you'd like to set up massage for your company as a one-day treat or as a recurring benefit, this contract lays out the details of our agreement.
